Initial Psychiatric Evaluation
Your first visit at Mental Alive is a full 60-minute appointment, not the rushed 15 minutes typical of telehealth platforms. That's enough time to actually understand who you are, what you've been struggling with, and what hasn't worked before.
During the evaluation, we'll review your symptoms, medical and psychiatric history, current life context, and treatment goals. By the end of the visit, you'll leave with a clear diagnostic picture and a personalized treatment plan that may include medication, lifestyle recommendations, therapy referrals, or specialty treatments like Spravato or Suboxone when appropriate.
If we determine you'd benefit from ongoing care, your follow-up schedule is built around your treatment, not a one-size-fits-all calendar.
Self-pay: $299 — Initial Evaluation (60 minutes)
Insurance accepted: Aetna, Florida Blue, Cigna/Evernorth, United Healthcare/Optum
Self-pay patients seeking ongoing care may qualify for our Mental Alive Care Membership for transparent monthly pricing.

Follow-Up & Medication Management
Once treatment begins, regular follow-up visits help us monitor your response, adjust medications when needed, and make sure your care continues to fit your life.
Most patients are seen every 2-4 weeks at first, then less often as symptoms stabilize. Patients on controlled medications, Spravato, or MAT may need more frequent visits. Your schedule is built around your treatment, not a fixed calendar.
At every follow-up visit, you'll see the same provider. No rotating clinicians, no starting over each time.
Self-pay: $149 — Follow-up visit (15-30 minutes)
Insurance accepted: Aetna, Florida Blue, Cigna/Evernorth, United Healthcare/Optum
Self-pay patients seeking ongoing care may qualify for our Mental Alive Care Membership for transparent monthly pricing.
SPRAVATO® (Esketamine) Treatment
An innovative treatment for adults with treatment-resistant depression.
Spravato® is an FDA-approved nasal spray treatment for adults with treatment-resistant depression (TRD) and for major depressive disorder with acute suicidal ideation. Unlike daily oral antidepressants, Spravato works through a different mechanism, and for many patients who haven't responded to standard medications, the difference is significant.
At Mental Alive, Spravato is administered in our private, REMS-certified treatment room by a board-certified Psychiatric Mental Health Nurse Practitioner. Each session includes the medication administration and a two-hour monitored observation period in a calm, comfortable setting designed for safety and privacy.
Spravato may be an option if:
-
You've tried two or more antidepressants without significant relief
-
Your depression interferes with daily life, work, or relationships
-
You're seeking a treatment supported by strong clinical evidence
-
You're willing to commit to the treatment schedule (initially 2 sessions per week, then weekly, then biweekly)
What to expect:
-
Comprehensive evaluation to confirm Spravato is appropriate
-
Insurance authorization support from our team
-
In-office administration in a private, monitored setting
-
Ongoing care coordination with your existing providers when appropriate
Insurance: Spravato is covered by most major insurance plans (Aetna, Florida Blue, Cigna/Evernorth, United Healthcare/Optum) when medical necessity criteria are met. Our team verifies your benefits before treatment begins.
Self-pay options available for patients without coverage. Pricing discussed during your initial consultation.

MAT / Suboxone Treatment
Confidential outpatient treatment for opioid use disorder.
Medication-Assisted Treatment (MAT) for opioid use disorder combines FDA-approved medication, primarily buprenorphine (Suboxone), with ongoing psychiatric care to help patients stabilize, reduce cravings, and rebuild their lives. It is the gold standard for outpatient OUD treatment, supported by decades of clinical evidence.
At Mental Alive, MAT is offered with the same care and dignity as any other psychiatric service. There is no separate "addiction clinic" environment, no group lobbies, no stigma. Visits happen in our private office or by telehealth, in confidential one-on-one sessions with the same provider every time.
MAT may be right for you if:
-
You are currently using opioids (prescription or non-prescription) and want help stopping
-
You have a history of opioid use disorder and need ongoing maintenance treatment
-
You've completed detox and need outpatient support to stay stable
-
You're transitioning from methadone or another MAT program
-
You're ready for confidential, judgment-free outpatient care
What to expect:
-
Initial evaluation to confirm MAT is clinically appropriate
-
Florida PDMP review and safety screening
-
Same-week induction available for most patients
-
Ongoing monthly visits with prescription management
-
Optional coordination with therapy or other support services
Insurance: Buprenorphine treatment is covered by most major insurance plans (Aetna, Florida Blue, Cigna/Evernorth, United Healthcare/Optum).
Initial evaluation and follow-up visits are billed through your insurance plan.
Self-pay: Standard self-pay rates apply ($299 initial evaluation, $149 follow-up). Same-week appointments are typically available.
Alcohol Use Disorder Treatment
Evidence-based outpatient care for adults ready to make a change.
Alcohol Use Disorder (AUD) is one of the most common and most underdiagnosed psychiatric conditions in the United States. Many people who would benefit from treatment never seek it, often because they fear judgment or assume their drinking "isn't bad enough" for medical help. It doesn't have to be.
At Mental Alive, AUD is treated as the medical condition it is, without lectures, without group sessions, without stigma. Treatment combines FDA-approved medications, primarily naltrexone (oral or Vivitrol injection), with psychiatric care to reduce cravings, support sobriety or moderation goals, and help you regain control. Visits are confidential, one-on-one, and held in our private office or by telehealth.
AUD treatment may be right for you if:
-
You drink more than you'd like and want help cutting back or stopping
-
You've tried to moderate or quit on your own without lasting success
-
Alcohol is interfering with your work, relationships, sleep, or health
-
You're concerned about the role alcohol plays in your life, even if others aren't
-
You're looking for medical treatment, not just willpower
-
You want a private, confidential alternative to AA or rehab programs
What to expect:
-
Initial evaluation to discuss your drinking patterns, goals, and health history
-
Medical screening (blood work and safety review) before starting medication
-
Prescription of naltrexone (oral) or coordination for monthly Vivitrol injection
-
Ongoing psychiatric care and medication adjustments
-
Optional coordination with therapy or other support services
A note on scope of care:
Mental Alive provides outpatient AUD treatment for medically stable patients. We do not provide medical detox or inpatient withdrawal management. If you are drinking heavily and at risk of withdrawal complications, we'll help you connect with the right level of care before starting outpatient treatment.
Insurance: AUD treatment is covered by most major insurance plans (Aetna, Florida Blue, Cigna/Evernorth, United Healthcare/Optum).
Initial evaluation and follow-up visits are billed through your insurance plan. Naltrexone prescriptions are filled at your preferred pharmacy and covered by most plans.
Self-pay: Standard self-pay rates apply ($299 initial evaluation, $149 follow-up). Same-week appointments are typically available.

ADHD Evaluation & Treatment
ADHD evaluations at Mental Alive are different from what you'll find at most cash-pay clinics. Florida has strict prescribing requirements for stimulant medications, and we follow a thorough, evidence-based diagnostic protocol, not a 20-minute questionnaire and a prescription.
The result: a confident, accurate diagnosis, treatment decisions that hold up under clinical scrutiny, and care that protects you long-term.
Every ADHD evaluation at Mental Alive includes:
-
A comprehensive 60-minute psychiatric assessment
-
Validated ADHD screening questionnaires
-
FDA-cleared objective computerized testing (20-30 minutes). Measures attention, impulsivity, and processing speed with hard data, not just self-report.
-
Florida PDMP review and safety screening as clinically indicated
-
A clear diagnostic conclusion and personalized treatment plan
-
Follow-up visit at 2 weeks to assess treatment response (included in self-pay bundle)
Two ways to get evaluated:
For patients with insurance:
Your initial psychiatric evaluation is billed through your insurance plan. The computerized ADHD test is a $175 self-pay fee, as it's not covered by most insurance plans.
We accept Aetna, Florida Blue, Cigna/Evernorth, and United Healthcare/Optum.
For self-pay patients — ADHD Evaluation Bundle: $375
All-inclusive package includes:
-
60-minute initial psychiatric evaluation
-
FDA-cleared computerized ADHD testing
-
Clinical interpretation and treatment recommendations
-
2-week follow-up visit
A savings of nearly $100 compared to paying for each service separately.
About stimulant medications:
Not every patient evaluated for ADHD requires or receives stimulant medication. After your evaluation, we'll review whether stimulants are clinically appropriate for you, or whether non-stimulant treatment options would be a better fit.
For patients prescribed stimulants, ongoing monitoring includes Florida PDMP review and periodic safety check-ins, protections that exist because stimulant prescribing in Florida is heavily regulated and we take that responsibility seriously.
